Job Description

Overview HealthDrive is currently seeking a Licensed Independent Clinical Social Worker (LICSW) to work Part-Time or Full Time (2-5 days per week) in nursing facilities across Greater Boston, MA area (work available in Chelsea, Malden, Saugus, and Danvers ). •

Responsibilities The Therapist will provide behavioral health services to the residents of nursing homes, assisted living facilities, and long-term care facilities within an assigned territory.
Essential Functions:
Complete psychosocial (initial) assessments Provide individual and group therapy Develop behavior plans for residents Recommend behavioral/therapeutic interventions to nursing home staff Enters patient information and required clinical documentation in EMR system Perform other duties as assigned Mobile model treatment of patients requires daily travel to skilled nursing facilities Qualifications Master's degree from an accredited college or university State professional LICSW license Valid driver's license Ability to work independently on a daily basis Excellent written, verbal, interpersonal and organizational skills Ability to use email, the internet, and to learn other healthcare related software
