$2,500.00 SIGN-ON BONUS!!! Part-Time Home Health Visits Licensed Practical Nurse Flexible Schedule and Work/Life Balance!! BAYADA Home Health has an immediate opening for a part-time Licensed Practical Nurse to perform home health visits in Assisted Living and Independent Living Communities, in Millis, Norwood, Westwood and Dedham. You will provide exceptional medical care to geriatric patients in these Senior Living Communities. Prior experience as a Licensed Practical Nurse is required. Licensed Practical Nurse (LPN)
Responsibilities:
Making home visits to clients living in senior living communities Performing assigned duties, including administration of medication, wound care, treatments, and procedures. Monitoring clients' conditions; reporting changes to Clinical Manager and Client Services Manager. Following up with, executing, and properly documenting doctors' orders. Performing client assessments as necessary. Case management and coordination. Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing electronic medical records on a touch pad tablet. Qualifications of a Licensed Practical Nurse (LPN): A current license as a Licensed Practice Nurse in the state of Massachusetts. Minimum of one year of recent sub-acute/rehab experience as an LPN. Base Pay:
$45.00 - $53.00 per point depending on qualifications.
