Job Description

Trilogy Health Services is seeking a Licensed Practical Nurse to provide compassionate care to seniors in our long-term and assisted living community. In this role, you'll administer medications, perform treatments, monitor changes in condition, and support RNs and physicians in delivering personalized care. You'll supervise CNAs, ensure regulatory and infection control compliance, and maintain accurate documentation. Responsibilities Provide direct nursing care to residents including medication administration, treatments, and monitoring vital signs Assist with care plans, documenting resident status and communicating changes to the RN and interdisciplinary team Support physicians and RNs during assessments, procedures, and follow-up care Educate residents and families on care plans, medications, and wellness practices Ensure compliance with safety, infection control, and regulatory standards in a long-term care setting Supervise and support nursing assistants, promoting teamwork and high-quality resident care Respond promptly to resident needs, emergencies, and call lights with compassion and professionalism Maintain accurate, timely electronic health records and documentation Required Skills Medication administration Wound care Electronic health records (EHR) Vital signs monitoring Care planning Infection control Geriatric nursing Patient and family education Emergency response Supervision of CNAs
