Minnesota-based senior living organization with accessible leadership. Know Who You're Caring For There's a clinical advantage to caring for the same people. When you've cared for someone repeatedly, you know that they're usually talkative at dinner. You know they normally walk independently. You know what their typical confusion looks like. You know how much they normally eat. So when something suddenly changes, you notice. You aren't comparing them to a chart. You're comparing them to the person you know. That familiarity can help you recognize changes earlier, communicate better with providers, and help prevent a small concern from becoming a much bigger one. What Your Shift Could Look Like As an LPN, you'll: Administer medications and treatments according to provider orders. Perform resident assessments and recognize changes in condition. Provide nursing care for residents with chronic medical conditions and cognitive impairment. Communicate concerns with providers and nursing leadership. Maintain accurate clinical documentation and medication records. Support individualized resident service plans. Provide clinical guidance to caregivers and medication staff. Communicate with residents and families. Coordinate with physicians, pharmacies, therapists, home health, hospice, and other healthcare partners. Promote resident safety, dignity, independence, and quality of life . Your LPN Experience Transfers If you've worked in: Long-Term Care. Skilled Nursing. Rehabilitation. Assisted Living. Memory Care. Home Health. Hospice. Clinic Nursing. Geriatric Care. You may already have much of the experience we're looking for. You'll need an active Minnesota Licensed Practical Nurse (LPN) license , strong communication skills, good clinical judgment, and the ability to work confidently within your scope of practice. Most importantly, we're looking for someone who still cares about doing nursing well . Maybe You Don't Need to Leave Nursing. Maybe You Need a Different Nursing Environment.
