Job Description
Overview Join our dynamic healthcare team as a Licensed Practical Nurse (LPN), where your expertise will directly impact patient care and well-being. In this vital role, you will provide compassionate nursing support across diverse clinical settings, ensuring high-quality care for individuals of all ages. Your skills in patient assessment, medication administration, and clinical documentation will help foster a safe, efficient, and healing environment. Duties Perform comprehensive patient assessments including vital signs, physical examinations, and health histories to inform care plans. Administer medications, injections, IV infusions, and other treatments following strict protocols and safety standards. Document patient information accurately using Electronic Health Records (EHR) systems such as Epic, Cerner, or Athenahealth. Assist with diagnostic evaluations, specimen collection, and processing to support accurate diagnoses. Provide specialized care for patients with complex needs such as dementia, Alzheimer's disease, or those requiring hospice or palliative care. Manage airway procedures, catheterizations, tube feedings, and ventilator support as needed for critical or long-term care patients. Collaborate with interdisciplinary teams in settings like skilled nursing facilities, hospitals—including Level I Trauma Centers—and outpatient clinics to ensure seamless patient care. Requirements Valid LPN license in good standing with the state licensing board. Proven experience working in diverse healthcare environments such as hospitals, nursing homes, assisted living facilities, or home care. Strong knowledge of medical terminology, anatomy, physiology, and infection control practices. Experience with EMR/EHR systems including Epic, Cerner, or eClinicalWorks; proficiency in medical documentation is essential. Competency in performing IV insertions, venipuncture (phlebotomy), medication administration, and basic life support (BLS). Familiarity with long-term care regulations and geriatric patient needs; experience working with individuals with disabilities or developmental delays is a plus. Ability to communicate effectively with patients and caregivers while maintaining confidentiality according to HIPAA policies.
