Responsibilities:
Responsibilities Provide direct patient care in the home setting in accordance with the established plan of care. Perform skilled nursing tasks, including medication administration, wound care, and treatments as ordered. Monitor and document vital signs, symptoms, and patient responses to care Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support as needed Communicate effectively with the Registered Nurse (RN), physicians, and interdisciplinary team regarding patient status and changes Ensure accurate and timely documentation in compliance with agency policies and state regulations Educate patients and families on disease processes, medications, and care plans Maintain a safe and clean environment while following infection control and safety protocols Respond appropriately to emergencies and changes in condition Support continuity of care by attending required case updates and communication with supervisors
Pay:
$32.00 - $45.00 per hour
