We are seeking a motivated professional to join our team in a full-time role. This position offers a family-focused work environment with no overnights or holidays and only occasional Saturdays .

Key Responsibilities:

Interpret and carry out provider orders while maintaining high standards of patient care Coordinate patient services within the hospital and community; document all care accurately in the electronic medical record (EMR) Demonstrate a compassionate and culturally sensitive approach to patients and their families Assist with patient and family education to support understanding of care plans and treatments Obtain and accurately record patient histories and vital signs Review and update medication lists at each visit; prepare prescription refill requests for provider approval and electronic transmission Support efficient communication between patients, families, hospitals, and providers (including telephone coordination) Complete daily and monthly nursing checklists and logs; promptly report abnormalities to the Nursing Supervisor Contribute to a team-oriented environment in a community health setting serving diverse populations regardless of ability to pay Bilingual skills are a plus, to support diverse patient populations Interested in joining a supportive and collaborative team?
