Job Description

Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality patient care across diverse healthcare settings. The ideal candidate will possess a strong foundation in nursing practices, excellent communication skills, and a commitment to enhancing patient well-being. As an LPN, you will work closely with multidisciplinary teams to deliver safe, effective, and personalized care, ensuring compliance with all regulatory standards and organizational policies. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and medical history documentation using electronic health record (EHR) systems. Administer medications, injections, IV infusions, and perform catheterizations following sterile techniques and safety protocols. Assist with diagnostic evaluations such as specimen collection and processing, medical imaging preparation, and basic life support procedures. Support patients with activities of daily living, including mobility assistance, tube feeding, wound care, and dementia or Alzheimer's care. Collaborate with physicians, case managers, and caregivers to develop and implement individualized care plans aligned with long-term care regulations and best practices. Maintain accurate medical records in compliance with HIPAA confidentiality policies while ensuring proper documentation of patient care activities. Participate in discharge planning, patient education on medication administration, pain management strategies, and health coaching to promote recovery and wellness. Experience Proven experience working as an Licensed Practical Nurse in various healthcare environments such as hospitals, nursing homes, assisted living facilities, or home & community care settings. Demonstrated proficiency with EMR/EHR systems like Epic or Cerner for documentation and patient management. Prior experience in critical care areas such as ICU or Level I Trauma Centers is highly desirable. Knowledge of
ICD-9/ICD-10
coding for accurate medical documentation and billing processes. Familiarity with specialized care areas including geriatrics, dementia/memory care, hospice care, infectious disease management, pediatric or neonatal units is advantageous. Experience working with individuals with disabilities or developmental challenges enhances your ability to deliver comprehensive patient-centered care. Certification in Basic Life Support (BLS), CPR, and sterile processing techniques is required to ensure safety standards are met across all clinical procedures.
