Responsibilities:

Provide direct patient care to residents under the supervision of RNs including assistance with activities of daily living (ADLs), vital signs, and medication administration Develop and implement individualized care plans, in collaboration with the interdisciplinary team, to meet the unique needs of each resident Participate in care conferences, including discharge planning and family meetings, to ensure comprehensive care and support Maintain accurate and detailed records of patient care, including medication administration, vital signs, and treatment plans Collaborate with other healthcare professionals to ensure continuity of care and effective communication Perform vital signs and other clinical tasks as needed Assist with maintaining a safe and clean environment, adhering to infection control standards About the
