Collaborate with healthcare teams to develop and implement individualized care plans. Maintain accurate medical documentation and records in compliance with HIPAA regulations. Educate patients and their families about health management, medication administration, and treatment plans. Participate in triage and emergency response situations when required. Uphold infection control protocols and ensure a safe environment for patients and staff. Qualifications Valid LPN license in the state of practice. Experience in long-term care is preferred. Strong skills in patient assessment, medication administration, and basic life support techniques. Ability to work collaboratively within a multidisciplinary team while demonstrating excellent communication skills.
