Your Core Responsibilities:
Deliver compassionate, person-centered health care to individuals in residential settings. Administer medications and treatments as prescribed; monitor the patient's health status and report any changes to the RN. Provide personal care, support daily living routines, and promote independence. Coordinate and accompany individuals to medical appointments; ensure follow-up care is carried out. Assist the RN with developing nursing care plans, health-related goals, and staff training. Respond to emergencies and behavioral incidents, utilizing SCIP-R strategies. Act as shift leader when assigned and collaborate with interdisciplinary teams. Maintain accurate documentation in compliance with agency and regulatory requirements. Support a clean, safe, and welcoming home environment.
What You Bring:
Education & Licensure:
Current New York State Licensed Practical Nurse (LPN) license in good standing.
Certifications:
CPR and First Aid (or ability to obtain); SCIP-R Level II certification or willingness to complete training.
Requirements:
Valid NYS driver's license and ability to meet Lifetime Assistance's Vehicle Operator Requirements.
Experience & Skills:
Strong communication, problem-solving, and decision-making skills; ability to follow medical directions and adapt to changing care needs.
Physical Requirements:
Ability to lift up to 35 pounds, respond to emergencies, and perform frequent physical activity.
