About the Role We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our multidisciplinary team. As an LPN, you'll play a vital role in supporting individuals through recovery and withdrawal management. Under the supervision of RNs and Licensed Medical Practitioners, you will provide safe, evidence-based care in accordance with the Oregon Nurse Practice Act (OAR 815-045-0000). What You'll Do Implement care orders from authorized healthcare providers. Use clinical judgment and question unclear or unsafe orders. Administer medications accurately (using the "Five Rights" standard). Perform vital signs assessments, CBG testing, and weight monitoring. Provide assistance with daily living activities and patient hygiene. Report significant changes in patient condition to supervising staff. Collaborate with RNs, LMPs, and behavioral health staff. Onboard new residents and confirm intake medication orders. Complete discharge documentation and manage medication returns. Keep exam spaces clean and equipment sanitized and stocked. Support patients during detox and withdrawal with medical oversight. Monitor patients for withdrawal symptoms and vital signs. Administer withdrawal management medications and document interventions. Collect lab specimens and maintain specimen handling procedures. Assist with patient intake, discharge, meals, hydration, and ambulation. Uphold confidentiality and ethical nursing standards. Participate in staff orientation and training as needed. Maintain personal and department objectives (OKRs). Support a positive team culture focused on patient outcomes. Communicate effectively across clinical teams and during shift changes. What We're Looking For Graduate of an accredited School of Nursing. Documented experience with mental status charting, medication effectiveness, and changes in patient medical condition. Current Oregon LPN License, required CPR Certification, required Valid Driver's License, required CE in Addiction Treatment, Substance Abuse, or Trauma-Informed Care highly encouraged. Proficiency in bloodborne pathogen safety and emergency response protocols. Strong communication, documentation, and teamwork skills. Ability to maintain professional boundaries and confidentiality. Occasionally climb stairs and lift/move objects up to 10 lbs. Frequently stand, sit, and repeat hand or wrist motions. Constantly communicate and assess documentation accuracy. Occasionally work in moderate noise environments.
