Duties Conduct thorough patient assessments including vital signs, physical examinations, and medical history documentation. Administer medications and injections following proper aseptic techniques and medication protocols. Monitor patients' physiological status using advanced equipment such as ventilators, IV infusion devices, and catheters. Manage complex care needs including wound suturing, ostomy care, tube feeding, and infection control measures. Collaborate with interdisciplinary teams to develop and implement individualized discharge plans aligned with patient needs and preferences. Educate patients and families on disease management, medication administration, nutrition, and post-discharge care instructions. Document all patient interactions accurately within electronic health records (EHR systems) like Epic or Cerner while adhering to HIPAA regulations. Participate in quality improvement initiatives aligned with NCQA standards and other regulatory requirements. Provide specialized care for diverse populations including pediatrics, geriatrics, behavioral health patients, or those with complex chronic conditions such as dementia or Alzheimer's disease. Assist in diagnostic evaluations involving laboratory specimen collection/processing and medical imaging procedures such as sonography or mammography when required. Maintain sterile processing protocols for equipment sterilization and ensure infection control standards are consistently met.
