Job Description

LPN Location:
Warm Springs Health and Wellness Center, Warm Springs, OR Shift:
Clinic hours are generally:
Monday, Tuesday, Thursday, Friday:
8:00 AM 5:00
PM Wednesday:
12:00 PM 5:00 PM Entitled to a one-hour lunch period The clinic is closed on federally recognized holidays and during emergency or administrative closures.

PTO:
5 days after 6 months, an additional 5 days at 1 year of service Accrued sick days: 1 hour for every 36 hours worked 11
Paid Holidays:
Following the Government calendar throughout the year LPN responsibilities include, but are not limited to: Performing patient health assessments and risk assessments as permitted within scope of practice; Reviewing and documenting patient health history; Recording blood pressure and applicable screening information; Performing appropriate health screenings and assessments; Completing nursing assessments and monitoring patient health status; Performing nursing interventions and patient care within scope of practice; Provide medication administration and management within scope of practice and applicable orders; Providing patient education and preventive health education; Counseling patients regarding health, wellness, and disease prevention; Coordinating follow-up care and preventive care scheduling; Administering medication and treatments as authorized and within scope of practice;
Requirements:
Graduation from an accredited practical nursing program; Current, unrestricted LPN license in a State, District of Columbia, Puerto Rico, or U.S. Territory; Current certifications required for LPN practice, if applicable; Minimum of one (1) year clinical nursing experience preferred; Current Basic Life Support (BLS) certification.
Pay:
$40.00 per hour Expected hours: 40 per week
