In this vital role, you will provide high-quality patient care in a homebased environment. Your expertise will support patient recovery, comfort, and well-being through skilled nursing interventions, medication administration, and thorough assessments. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation. Administer medications, injections, IV infusions, and other treatments following strict protocols and safety standards. Monitor patients' conditions closely, recognize signs of acute pain or deterioration, and escalate concerns to physicians or advanced practice providers promptly. Collaborate effectively with multidisciplinary teams to develop personalized care plans that meet regulatory standards like long-term care regulations and NCQA standards.

Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice. Proven experience working in diverse healthcare environments such as hospitals (including Level I Trauma Centers), nursing homes, skilled nursing facilities, or home care settings. Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/ICD-10), and documentation best practices within EMR/EHR systems like Athenahealth or eClinicalWorks. Demonstrated expertise in patient care areas including geriatrics, pediatrics (including neonatal ventilator management), behavioral health, infectious disease care, and acute pain management. Experience with IV insertion/infusion therapy, specimen collection/processing, catheter care, and first aid procedures. Ability to communicate effectively with patients of all ages—including those with disabilities—and caregivers while maintaining clinical confidentiality policies such as HIPAA. Must complete ODP training Knowledge of infection control protocols and safety standards essential for high-quality nursing practice in diverse settings. Pay:
$30.00 - $35.00 per hour Expected hours: 12.0 per week
