Job Description

Overview We are seeking a dedicated and energetic LVN (Licensed Vocational Nurse) or RN (Registered Nurse) specializing in Wound Care to join our dynamic healthcare team. This vital role offers an exciting opportunity to provide expert wound management and patient care across diverse settings, including long-term care, assisted living, hospitals, and outpatient clinics. Responsibilities Conduct thorough wound assessments, including measuring wound size, depth, exudate, and tissue condition to develop personalized treatment strategies. Implement advanced wound care techniques such as dressing changes, debridement, compression therapy, and application of topical agents. Manage complex cases involving diabetic foot ulcers, pressure injuries, surgical wounds, and burns with precision and compassion. Collaborate with multidisciplinary teams—including physicians, physical therapists, dietitians, and case managers—to coordinate comprehensive patient care plans. Educate patients and their families on wound management protocols, infection prevention measures, and lifestyle modifications to support healing at home. Document all patient interactions meticulously within Electronic Health Record (EHR) systems to ensure accurate medical records and compliance with HIPAA regulations. Monitor patient progress regularly through vital signs assessment, laboratory results review (including ICD-10 coding), and physiological evaluations to adjust treatments as needed. Requirements Valid LVN or RN license in good standing; additional certification in Wound Care (such as WCC or CWCN) is preferred but not mandatory. Familiarity with EMR/EHR systems for accurate documentation and case tracking. Strong knowledge of anatomy, physiology, infection control protocols, and sterile technique essential for effective wound treatment. Ability to perform detailed patient assessments including vital signs measurement, venipuncture (phlebotomy), catheter care, and injection administration when necessary. Excellent communication skills for educating patients on wound care procedures and collaborating effectively with healthcare teams. Experience working with diverse populations including individuals with developmental disabilities or dementia (Alzheimer's/memory care).
