Job Description

Job Overview We are seeking a dedicated and energetic Licensed Vocational Nurse (LVN) to join our compassionate healthcare team. In this vital role, you will provide high-quality patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise in nursing practices, patient assessment, medication administration, and clinical documentation will ensure our patients receive safe, effective, and personalized care. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care planning. Administer medications and injections accurately following prescribed protocols while monitoring for adverse reactions. Manage IV infusions and catheter care with precision, ensuring sterile techniques and patient safety. Assist with diagnostic evaluations such as specimen collection, processing, and medical imaging procedures. Provide specialized care for patients with complex needs including Alzheimer's disease, dementia, memory care, and developmental disabilities. Support acute pain management strategies and participate in emergency triage or urgent care situations as needed. Document all patient interactions thoroughly using Electronic Health Records (EHR) systems like Epic or Cerner while maintaining strict confidentiality policies under HIPAA regulations. Qualifications Valid LVN license in good standing with the appropriate state licensing board. Proven experience working in diverse healthcare environments such as hospitals (including ICU or Level I Trauma Centers), nursing homes, skilled nursing facilities, or home & community care settings. Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth for accurate medical documentation. Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and long-term care regulations. Experience with specialized areas such as hospice care, infectious disease management, ventilator support, tube feeding, or first aid is highly desirable. Ability to perform physical examinations and procedures like venipuncture, phlebotomy, catheterization, airway management, and sterile processing techniques. Excellent caregiver communication skills with the ability to work effectively with patients of all ages—including pediatrics and geriatrics—and their families.
