Medical, Dental, Vision, 401(k) match, PTO, and more What You'll Do Deliver Compassionate Hospice Care Provide direct patient care in accordance with the Nurse Practice Act, with a focus on end-of-life support Implement individualized plans of care under the supervision of a Registered Nurse Deliver effective pain and symptom management, monitoring and adjusting based on patient response Support patients with daily care needs while ensuring comfort, dignity, and safety Educate patients and families on disease progression, medications, and comfort measures Coordinate & Communicate Document care accurately and timely in the EMR Communicate changes in patient condition to the RN and care team Collaborate with interdisciplinary partners (RN, HHA, Social Work, Chaplain, Therapy) Participate in care planning and coordination to ensure holistic support Commit to Excellence Follow best practices and clinical standards in hospice care Maintain proper use of equipment and adhere to infection control protocols Participate in quality improvement initiatives and team development Continue professional growth through training and education What You Bring Active LPN/LVN license in the state of practice Current CPR certification 1-3+ years of nursing experience preferred (hospice, home health, or med-surg) Strong clinical judgment, communication skills, and emotional intelligence A genuine passion for patient-centered, end-of-life care Valid driver's license, reliable transportation, and auto insurance Who We Are Founded in 2021, VitalCaring is a leading home health and hospice provider with 100+ locations nationwide.
