Position Summary Advanced Pediatric Care Home Health is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to provide skilled nursing care to pediatric patients in their homes. The LVN will work under the direction of a Registered Nurse (RN) and physician to deliver high-quality, individualized care that promotes the health, comfort, and well-being of children with medical needs. Essential Duties and Responsibilities Provide direct nursing care to pediatric patients in a home setting. Monitor and assess patient conditions, vital signs, and response to treatment. Administer prescribed medications, treatments, and therapies. Perform skilled nursing procedures, including tracheostomy care, ventilator monitoring, G-tube care, and suctioning as appropriate. Document patient care accurately and timely in accordance with agency policies and state regulations. Educate and support patients and family members regarding care plans, treatments, and disease management. Observe and report changes in patient condition to the supervising RN and physician. Maintain a safe, clean, and therapeutic environment for patients. Participate in care plan development and interdisciplinary team meetings as needed. Ensure compliance with all HIPAA, state, and federal regulations. Qualifications Current Texas Licensed Vocational Nurse (LVN) license in good standing. Current CPR/BLS certification. Graduate of an accredited vocational nursing program. Pediatric nursing experience preferred. Home health experience preferred but not required. Strong assessment, communication, and organizational skills. Ability to work independently and make sound clinical decisions. Reliable transportation and valid driver's license. Knowledge, Skills, and Abilities Knowledge of pediatric nursing practices and home health regulations. Ability to establish positive relationships with children and families. Strong verbal and written communication skills. Ability to maintain confidentiality and professionalism at all times. Proficient documentation and time-management skills. Physical Requirements Ability to lift, move, and transfer patients as needed. Ability to bend, stoop, kneel, and stand for extended periods. Ability to travel to patient homes within the service area.
