STATEMENT OF RESPONSIBILITIES

This is a FLSA non-exempt position. The incumbent provides services to residents who live in Town Creek Assisted Living facility and must maintain safe conditions for the consumers and provide general care and supervision. The incumbent reports to the Director III and position duties require proceeding along under standard practices, referring questionable situations to the supervisor. Carrying out position duties requires analysis of facts and determining action using a wide range of procedures but within limits of standard practice. He/she is expected to perform within accordance with applicable professional ethics, state quality assurance standards, Department of Behavioral Health and Developmental Services regulations, Department of Social Services regulations as well as established Region Ten polices. MAJOR DUTIES

The essential functions of this job are starred below (•) under \"Major duties.\" 1. •Contact Benefit Specialist at DSS to consult pending applications for residents' benefits/Complete Auxiliary Grant and Medicaid Applications 2. Complete original AG applications if one was not submitted by the hospital. 3. Obtain needed verifications in order to get resident approved and track down correct localities if there is a dispute about which DSS locality should pay. 4. Complete annual AG renewal and send them to the proper DSS office. 5. Track Auxiliary grants for annuals reviews. 6. Schedule medical appointments and non-medical appointments/coordinate transportation to community and medical outings/complete necessary paperwork for appointments. 7. Send appointment requests to the PCP's and other medical professionals. 8. Review medical paperwork for medication changes and follow up appointments. 9. Complete pharmacy reviews. 10. Compete physician reviews every 6 months. 11. Help residents with cashing checks/obtaining their money upon request/fill out documentation to ensure all financial transactions are valid and covered 12. Obtain identification documents for residents (SSA Card, Birth certificate, insurance cards) 13. Work with residents and their insurance companies to help resolve issues that occur. 14. File appeals for medical claims. 15. Call hospitals and other providers to check claims and file for financial assistance 16. Contact SSA with residents and resident's payees to ensure residents are receiving their benefits and help resolve issues that evolve overtime. 17. Consult with insurance companies regarding coverage for appointments/obtain insurance cards when necessary. 18. Help residents track their financials through the lockbox and banking system 19. Review charts/file necessary paperwork in resident charts 20. Scan discharge resident charts into a thumb drive and store paper charts in filling cabinets. 21. Store resident's charts that are a year old in the storage room to allow more space up front for current residents 22. Thin resident's charts and file the information 23. Complete healthcare reviews with residents 24. Transport residents to medical appointments when all other transportation options are exhausted 25. As directed by the Director III, facilitate repair of durable and consumer equipment (glasses, hearing aids, wheelchairs, etc.). 26. •Acquires and maintains valid certification in First Aid, CPR, medication administration, non-aversive behavior management to comply with state/federal residential staffing requirements. 27. •Maintain confidentiality of all consumer care and personal information. 28. Participates in staff meetings as required for consumer and program planning, staff training, and evaluation. 29. Reads all agency communication. 30. Other job related duties as assigned by the Program Manager

QUALIFICATIONS

One (1) year of direct care work experience in a long-term care facility or residential program preferred. Medication Technician Certification to be obtained (Training will be provided.)

EDUCATION

At least two years towards a college degree in a human services field or

LPN. EXPERIENCE

Experience working in a assisted living facility or medical office preferred.

CERTIFICATION/LICENSE

RMA preferred In order to ensure the safe and efficient fulfillment of any travel on behalf of the board, a valid Virginia driver's license plus an acceptable driving record as issued by the Virginia Department of Motor Vehicles are required. For use of a personal vehicle to conduct Region Ten business, a certificate of valid personal automobile insurance must be provided.

KSAS:
