Job Description

HarmonyCares seeks a Medical Assistant Clinical Partner to support home-based primary and palliative care for medically complex, home-limited patients. In this field-based role, you'll accompany providers to patient homes, obtain vitals, perform basic clinical tasks, assist with exams and procedures, and document in the EHR. You'll help coordinate labs, diagnostics, telehealth visits, and follow-up care while educating patients and caregivers. Working within an interdisciplinary team, you'll ensure compassionate, patient-first service that reduces avoidable hospitalizations and improves outcomes.
