Job Description

Ozark Total Healthcare is seeking an experienced Medical Assistant to join our Chronic Care Management call-center team. This position provides structured monthly outreach, care coordination, care-plan support, and follow-up for patients with multiple chronic conditions. The Care Coordinator II works under established protocols and the clinical direction of an RN and the billing provider. What You'll Do Conduct scheduled CCM calls and document patient-reported symptoms, goals, barriers, adherence concerns, and progress. Review medication lists for completeness, document discrepancies, and route concerns for licensed-clinician or provider review. Reinforce provider-approved education and care-plan instructions. Coordinate appointments, referrals, records, pharmacies, community resources, transportation, and follow-up needs. Track hospital, emergency-department, specialist, home-health, and other care transitions. Recognize concerns requiring clinical review and promptly escalate them according to protocol. Document CCM activities and qualifying time accurately in eClinicalWorks and maintain assigned work queues. Provide professional, compassionate customer support to patients, caregivers, Ozark providers, and outside referring providers. Qualifications High school diploma or equivalent required. Medical Assistant certification or registration is preferred, including CMA, RMA, CCMA, or NCMA. Candidates without certification may be considered with at least five years of recent, verifiable experience working as a Medical Assistant in primary care, family medicine, internal medicine, or a comparable ambulatory clinic. Strong knowledge of medical terminology, common chronic conditions, medications, primary-care workflows, and electronic health records. Excellent telephone communication, organization, documentation, teamwork, and patient-service skills. Experience with eClinicalWorks, Medicare patients, chronic disease outreach, or care coordination is strongly preferred.
