About the Role DermAssure is an AI-assisted dermatology platform providing onsite dermatologic care to residents of assisted living and skilled nursing facilities in partnership with MDCS Dermatology. We are looking for a detail-oriented Care Coordinator to serve as the operational backbone of our onsite clinic program across assisted living communities on Long Island. Responsibilities Schedule and coordinate dermatology visits for residents, working directly with facility staff and clinical providers Maintain and update the patient tracker for each visit — logging Rx status, biopsy tracking, follow-up needs, and encounter notes Coordinate prescription transmission to the facility pharmacy and nursing staff, and confirm receipt Track biopsy orders and results, and communicate status updates to the clinical team Follow up with residents, family members, or nursing staff regarding outstanding clinical needs Schedule and confirm follow-up appointments at the facility or offsite as directed by the provider Serve as the primary point of contact between the dermatology team, the facility, and the pharmacy Ensure all patients are seen in a timely manner and that no follow-up items fall through the cracks Qualifications 1-2 years of experience in a medical office, care coordination, or healthcare administrative role Strong organizational skills and attention to detail Comfortable working in a fast-paced clinical environment with elderly patients Proficiency with basic software tools (email, spreadsheets, scheduling platforms) Experience in dermatology, long-term care, or assisted living is a plus CNA, MA, or similar certification preferred but not required What You'll Be Part Of You'll be working at the intersection of clinical care and technology, supporting a growing dermatology program that brings specialist care directly to underserved elderly residents across Long Island.
