Job Description

Primary Responsibilities Clinical (Attending Physician) Responsibilities Provide comprehensive medical management to SNF residents, including evaluation, diagnosis, treatment, and ongoing follow-up. Round regularly in assigned facilities, maintaining timely and accurate documentation in the EMR. Collaborate closely with the Trio in-house Nurse Practitioner, offering guidance, oversight, and joint care planning to ensure continuity and adherence to best practices. Participate in interdisciplinary team meetings, care plan conferences, and family communications as needed. Support facility quality and utilization objectives by focusing on proactive, preventive, and patient-centered care. Ensure compliance with Medicare, facility policies, and Trio Health Clinicians' clinical standards. Administrative (Medical Director) Responsibilities Serve as the facility's primary medical liaison, supporting leadership and staff in achieving optimal quality and regulatory outcomes. Lead and participate in Quality Assurance & Performance Improvement (QAPI) initiatives and policy reviews. Provide clinical guidance to staff regarding infection control, medication management, and chronic disease management protocols. Review and address trends in readmissions, infection rates, and clinical performance; develop and support corrective action plans. Assist with survey readiness, compliance monitoring, and staff education. Support onboarding and orientation of new clinical providers working in the facility. Foster collaboration and communication between all disciplines to enhance resident outcomes and satisfaction. Qualifications Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO). Active Illinois medical license and DEA registration. Board Certification in Internal Medicine, Family Medicine, or Geriatrics preferred. Minimum of three years of experience in post-acute or long-term care settings. Prior experience as a Medical Director or in facility leadership strongly preferred. Strong communication, leadership, and teamwork skills. Compensation & Structure Variable Compensation (VC): 70% of patient visits based on expected collections.
Medical Director Stipend:
Additional fixed monthly amount per facility overseen (amount TBD based on scope and census). No paid time off or paid holidays. This structure reflects the dual focus on direct patient care and administrative oversight while maintaining alignment with Trio's value-based care model.
