CERTIFICATES, LICENSES, REGISTRATIONS

Must have by date of hire and maintain current throughout employment:

License to practice medicine by the State of New Mexico with ability to obtain admitting privileges and active staff membership at local hospital, if applicable.
DEA license and New Mexico Controlled Substance Registration (CSR).
Medicare PIN and UPIN numbers.
Medicaid number.
Have or be able to obtain BLS/CPR or ACLS certification within 30 days of hire.

SCREENING REQUIRMENTS

According to PMS Human Resources Policy 205.

006, \"Healthcare Personnel Testing & Immunization Requirements,\" this position must comply with required testing and immunizations. Testing and immunizations including TB, Hepatitis B (consent or waiver), Tdap, MMR, and Varicella, will be available at the employee's PMS health center. Influenza vaccines will be required according to dates set by the CDC and NMDOH. PHYSICAL DEMANDS

Travel by car, bus or airplane may be required associated with attendance at conferences, meetings and other duties carried out at distant locations in and out of state. Some overnight travel may be required.
Ability to stand, bend, stoop, squat/kneel, to perform duties on a daily basis is required.
This position requires the operation of office equipment, including computers. Adequate hand, wrist, and fine finger movement is required to manipulate equipment.
Frequently required to lift and/or move up to 10 pounds and occasionally required to lift and/or move up to 30 pounds.
Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
Position requires hearing ability in order to hear / talk with internal and external contacts face-to-face and to receive information over the phone.
