JOB SUMMARY
This position is responsible for processing incoming requests for patient medical records, ensuring accurate, timely, and compliant release of health information in accordance with HIPAA regulations and organizational policy. In addition, this role serves as a key point of contact between the organization, patients, healthcare providers, attorneys, insurance companies, and other authorized requesters.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Receive, log, and process requests for patient medical records from patients, providers, attorneys, insurers, andother authorized parties. 2. Verify requester identity and authorization (e.g., signed release forms, subpoenas, court orders) beforereleasing any records. 3. Retrieve records from electronic health record (EHR) systems and/or physical charts. 4. Review records for completeness and accuracy prior to release. 5. Redact or withhold protected information not authorized for release, by HIPAA and state law. 6. Track and manage request queues to ensure records are released within required turnaround times. 7. Maintain accurate logs of all disclosures, including who received records and when. 8. Communicate with requesters regarding status, missing authorizations, or fees. 9. Process record requests via mail, fax, secure email, or patient portal, as applicable. 10. Maintain strict confidentiality of all patient health information. 11. Stay current on HIPAA, state privacy laws, and organizational release-of-information policies. 12. Collect applicable fees for record copies in accordance with state regulations. 13. Other related duties as assigned by manager or designee.
JOB REQUIREMENTS AND QUALIFICATIONS
Education:
High school diploma or equivalent required. Associate degree in related health information management or related field helpful. Experience Minimum of one (1) year of experience in medical records, health information management or a health care administration role preferred. Proficient experience with Microsoft Office including Word, Excel, Outlook, Teams, and CPR+ software system preferred.
Skills and Competencies:
Knowledge of HIPAA and other applicable privacy regulations. Familiarity with EHR/EMR systems (e.g., Epic, Cerner, etc.) Strong attention to detail and organizational skills. Ability to handle sensitive information with discretion. Good communication skills, both written and verbal.
