Job Description

Medical Records Coordinator Chandler Care Center, Chandler, OK Job Summary:
We are seeking a highly organized and detail-oriented Medical Records Coordinator to join our team at Chandler Care Center. As a Medical Records Coordinator, you will be responsible for maintaining accurate, complete, and confidential resident medical records in accordance with federal, state, and facility regulations. Essential Duties and Responsibilities:
Maintain resident medical records in an organized, accurate, and confidential manner Assemble, prepare, and audit medical records for completeness and accuracy File physician orders, progress notes, laboratory reports, and other clinical documentation promptly Process new admissions, transfers, discharges, and deceased resident records Ensure all required documentation is completed according to facility policy and regulatory standards Monitor records for missing signatures, incomplete documentation, and physician authentication Coordinate the release of medical information in compliance with HIPAA and applicable privacy laws Maintain electronic health records (EHR) and paper records, as applicable Prepare records for audits, surveys, legal requests, and insurance reviews Archive inactive records and oversee record retention and destruction according to state and federal guidelines Assist nursing staff with locating, retrieving, and organizing resident records Maintain confidentiality of all resident information at all times Participate in quality assurance and performance improvement activities Perform other duties as assigned by the
Administrator or Director of Nursing Qualifications:
High school diploma or GED required Medical Records certification preferred but not required Experience in healthcare, skilled nursing, or long-term care preferred Knowledge of HIPAA regulations and medical terminology Experience with Electronic Medical Record (EMR/EHR) systems preferred Proficient in Microsoft Office applications Excellent organizational, communication, and time-management skills Ability to handle confidential information with professionalism
Physical Requirements:
Ability to sit, stand, and walk for extended periods Frequently lift, carry, and move files weighing up to 25 pounds Manual dexterity sufficient for keyboarding and handling medical records Ability to reach, bend, stoop, and file records in cabinets Visual ability to read printed and electronic documents
Knowledge, Skills, and Abilities:
Strong attention to detail and accuracy Excellent organizational and filing skills Ability to prioritize multiple tasks and meet deadlines Strong customer service and interpersonal skills Ability to work independently and as part of a multidisciplinary healthcare team Knowledge of state and federal regulations governing medical records in long-term care settings
Work Environment:
