Think Positive Counseling and Coaching Services, LLC is Core Mental Health Agency dedicated to providing compassionate, high-quality behavioral health services to children, adolescents, adults, and families throughout Georgia. We are seeking a highly organized, compassionate, and customer-focused Behavioral Health Medical Records Clerk to join our growing team. Position Summary The Medical Records Clerk is responsible for maintaining accurate electronic health records, creating new client records, processing referrals, scanning and indexing documentation, monitoring chart completeness, and supporting compliance with HIPAA, Georgia DBHDD, and CARF standards, Medicaid, Medicare, and commercial Payer requirements. This position works closely with clinical and administrative staff to ensure timely, accurate, and confidential medical record management. Essential Duties and Responsibilities Create and maintain electronic client records in the EHR. Process incoming referrals and establish new charts. Scan, upload, index, and file assessments, treatment plans, service orders, progress notes, nursing assessments, psychiatric evaluations, and discharge documentation. Review charts for missing signatures, diagnoses, dates, and required documentation. Track expiring assessments and treatment plans. Notify clinicians of incomplete documentation and track completion. Prepare records for internal and external audits. Process requests for release of information in accordance with HIPAA. Maintain confidentiality of Protected Health Information (PHI). Assist the Quality Assurance department with chart audits. Respond to medical record requests promptly. Maintain record retention according to agency policy. Perform other duties as assigned. Marginal Functions Assist with reception coverage as needed. Participate in staff meetings and training. Other duties assigned. Performance Standards & Productivity Expectations Maintain chart accuracy. Complete assigned chart reviews within established deadlines. Respond to record requests within one business day when possible. Maintain confidentiality with zero unauthorized disclosures. Demonstrate professionalism and dependable attendance. Knowledge, Skills, and Abilities (KSAs) Knowledge of HIPAA, Medicaid/Medicare documentation, DBHDD and CARF standards. Knowledge of EHR systems (eCase preferred). Strong organizational, communication, and time management skills. Ability to prioritize multiple tasks and work independently. Microsoft Office proficiency (Word, Excel, Outlook) Attention to detail Professional written and verbal communication Customer service Critical thinking Team collaboration Microsoft Office proficiency (Word, Excel, Outlook) Minimum Qualifications High school diploma or GED Valid Georgia driver's license One year of medical records, healthcare administration, or behavioral health experience preferred. Experience with electronic health records (eCase preferred) Ability to pass background screening. Preferred Qualifications LPC, LCSW, LPN or RN. One year of behavioral health or medical records experience. eCase experience. Medical terminology knowledge. Physical Requirements Prolonged periods of sitting at a desk and working on a computer. Frequent telephone communication. Ability to lift up to 20 pounds occasionally. Ability to perform repetitive hand and keyboard movements.
