Job Description
JOB TITLE
Health Information Specialist SUMMARY:
Establishes and maintains medical records on consumers in a neat, orderly manner. Prepares consumer records for requests of information.
EDUCATION AND EXPERIENCE
Education:
High School Diploma/GED preferred. Coursework in general office and computers preferred.
Licensure:
N/A Certification:
N/A Experience:
Experience in customer service preferred.
Knowledge/Skills:
General office skills. Verbal/written communication skills. Skilled in use of all major computer applications. Punctual with consistent/reliable attendance history.
EQUIPMENT
Computer, fax, copier, and any other equipment required to perform the functions of the position.
MAJOR DUTIES AND RESPONSIBILITIES
1. Prepare and maintain medical and consumer records in a neat and appropriate manner. 2. Respond to incoming emails, internal requests and telephone calls for medical records in a timely manner. 3. Ensure that accurate and complete information is collected on all request for medical records. 4. Prepare consumer records for requests of information. 5. Sort and scan documents in the consumers chart timely and accurately. 6. Process location specific reports on a routine basis (i.e. subpoena log, medical records request log etc.) 7. Distribute internal and external mail within the location and prepare outgoing mail daily. 8. Assures confidentiality of all medical records while following established policies and procedures for release of information. 9. Performs other administrative duties such as locking/unlocking doors, typing, filing, faxing and collecting documents to be shredded, etc. 10. Attends and participates in regularly scheduled staff meetings and in-services and individual program planning staffing meetings, as needed. 11. All other duties as assigned.
