Job Description

The Health Information Specialist is responsible for maintaining accurate, complete, secure, and accessible patient health information in accordance with organizational policies and applicable federal and state regulations. This position performs a variety of Health Information Management (HIM) functions, including scanning and indexing records, maintaining electronic and paper medical records, processing release-of-information requests, and ensuring the integrity and confidentiality of patient information.
Essential Duties and Responsibilities:
Maintain and organize patient medical records in electronic and/or paper formats. Scan, index, upload, and accurately route medical record documents within the Electronic Health Record (EHR). Review medical records for completeness, accuracy, and appropriate documentation. Process requests for release of medical information in accordance with HIPAA and applicable laws and policies. Verify appropriate authorization before releasing protected health information (PHI). Retrieve medical records and documentation for patients, providers, departments, and authorized outside entities. Maintain confidentiality and security of all patient health information. Identify and assist with correcting duplicate or inaccurate patient records. Process incoming and outgoing medical records, faxes, correspondence, and other health information. Maintain appropriate logs and documentation related to medical record requests and releases. Assist with medical record audits and quality assurance activities. Communicate effectively with patients, physicians, clinical staff, insurance companies, and outside healthcare organizations. Follow established retention, storage, and destruction procedures for medical records. Maintain working knowledge of HIPAA, medical terminology, and applicable health information regulations. Perform other duties as assigned. High school diploma or equivalent required. Previous healthcare, medical records, HIM, clerical, or data-entry experience preferred. Knowledge of medical terminology and electronic health record systems preferred. Strong attention to detail and organizational skills. Ability to maintain strict confidentiality of protected health information. Strong computer and data-entry skills. Excellent communication and customer service skills.
