In this vital role, you will assist with medication administration and patient care, ensuring residents receive safe, timely, and effective treatments. Your expertise will support our residents' well-being across various care settings, including assisted living, nursing homes, hospitals, and home care environments. This position offers an opportunity to make a meaningful difference in the lives of individuals with diverse needs, including those requiring memory care, dementia support, hospice care, and specialized Alzheimer's care. The ideal candidate is energetic, detail-oriented, and committed to providing high-quality patient care while adhering to all safety and privacy regulations. Duties Safely administer medications according to physician orders and established protocols using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems such as Epic or Cerner Monitor vital signs regularly—blood pressure, pulse, respiration rates—and document findings accurately in medical records Assist residents with activities of daily living, including hygiene, mobility, feeding (including tube feeding), catheter care, and other personal care tasks Observe and report changes in residents' condition or behavior related to memory loss, dementia, or other cognitive impairments Maintain strict compliance with HIPAA regulations to protect patient privacy and confidentiality during all documentation and communication Support residents with medical procedures requiring physiology knowledge or anatomy understanding Collaborate with nursing staff and healthcare providers to ensure comprehensive patient care plans are followed effectively Participate in ongoing training on medical terminology, medication administration techniques, and emergency response procedures Experience Prior experience working in assisted living facilities, nursing homes, hospitals, or homeplus & community care environments is highly preferred Demonstrated knowledge of dementia, Alzheimer's disease, memory care strategies, hospice care principles, and caregiving best practices Familiarity with EMR/EHR systems such as Epic or Cerner for accurate medical documentation Experience working with individuals with disabilities or developmental challenges is advantageous Strong understanding of anatomy and physiology relevant to medication administration and patient monitoring Previous roles involving medical records management, vital signs collection, catheter or tube feeding assistance are beneficial Certification as a Medication Aide from an accredited program is required; ongoing training in medical terminology and HIPAA compliance is essential .
