The ideal candidate will have experience working with diverse populations, including seniors, individuals with disabilities, and those with neurodevelopmental or cognitive disorders. This role offers an opportunity to make a meaningful difference in clients' lives by promoting health, independence, and well-being through personalized care and support. Duties Assist clients with Activities of Daily Living (ADLs), such as bathing, grooming, dressing, and mobility assistance Monitor patient health status by observing vital signs, patient condition, and behavioral changes Support medication administration following proper protocols and HIPAA regulations Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and preparation Provide transportation assistance for medical appointments, errands, or community activities Maintain detailed care plans and document client progress accurately and confidentially Support residents with dementia or Alzheimer's disease through specialized memory care techniques Utilize patient transfer methods such as Hoyer lifts or manual handling to ensure safe mobility Assist with light housekeeping tasks including cleaning client living areas to promote a safe environment Observe and report any safety concerns or changes in client condition to healthcare professionals or family members Participate in ongoing training related to patient care, safety protocols, and HIPAA compliance Skills Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable Strong knowledge of patient observation, monitoring vital signs, medication administration, and care plans Experience working with individuals with autism, developmental disabilities, intellectual disabilities, or neurodevelopmental disorders Familiarity with dementia care, Alzheimer's care, memory care techniques, and geriatrics principles Ability to perform personal care assistance including assisting with ADLs and manual handling techniques such as Hoyer lifts and patient transfers Knowledge of HIPAA regulations to ensure confidentiality of client information CPR and First Aid certification or willingness to obtain upon hire Excellent communication skills for effective interaction with clients, families, and healthcare teams Compassionate demeanor with a strong commitment to providing respectful and personalized care Ability to work independently while following established protocols for patient safety and well-being This role is vital in supporting the health and independence of clients across various settings.
