Description of Services:

Contractor agrees to provide the Company with at least 10 hours each month, but shall not work more than 25-30 hours per month unless Contractor obtains written authorization from an officer of the Company. Purpose of this

Role:

Advocating and working towards assisting individuals by facilitating plans, goals, recommendations, and positive supports necessary to assist the individual in progression towards their desired outcomes as it relates to the behavioral support needs of the individuals served. Scope of Services Screening and evaluating individuals in terms of individuals' behavior, affective development, and activities as related to the scope of practice for behavioral services. Oversight of all individuals' progress toward identified needs that translate into appropriate goals and activities. Participate as a lead clinician within Coastal Connections Positive Behavioral Supports Team. Work in collaboration with other interdisciplinary team members to create plans to support the individuals served. Participate in quarterly interdisciplinary team meetings. Reviews and signs required documents. Serves as a consultant to staff and team members addressing questions related to behaviors as needed. Provides education and training to staff for carryover of supports. Prepares and maintains work setting, materials, and supplies in accordance with health and safety requirements and agency policies Contractor agrees to follow all applicable policies and procedures of Coastal Connections, Inc.

Note:

Currently, Coastal Connections currently does not accept members with intensive behavior support plans. Support is needed around behavior that is not an imminent risk. Contractor shall maintain at her/his own expense all professional licenses required by the Commonwealth to perform the Services. Contractor shall be responsible to immediately notify the Company in the event of the expiration or termination of all applicable licenses, or any disciplinary or investigations implemented by any state agency. Contractor shall maintain all commercial general or professional liability insurance during the term of the Agreement (for licensed clinicians).

Qualifications:

To fill this requirement for Day Hab, the candidate must meet one of the following criteria: The behavioral specialist must have one year's relevant work experience in developing behavioral programming for individuals. May be any of the following:

BCBA ABA

Therapist LCSW Masters or doctorate level teacher with certification in Special Ed.
