Our work extends beyond the traditional healthcare setting. We believe meaningful improvements in health require meeting people where they are, understanding the barriers they face, and connecting clinical care with prevention, education, community resources, and ongoing support. We are seeking a California-licensed Nurse Practitioner (NP) who is local to the San Francisco Bay Area, preferably within reasonable travel distance of Antioch and surrounding communities, and who shares our commitment to health equity and community-based care. Position Overview The Part-Time Contract Nurse Practitioner will provide direct patient care as part of Centered Care Wellness's nonprofit and community health programs. This is a hybrid position combining telehealth with in-person care. Visits may take place in patients' homes, community settings, outreach events, or other locations where Centered Care Wellness provides services. The ideal candidate is comfortable working outside of a traditional clinic model and understands that transportation, financial challenges, mobility, health literacy, social circumstances, and difficulty navigating the healthcare system can all affect a person's ability to receive appropriate care. This role is particularly well suited for an NP who values prevention, patient education, relationship-based care, and working collaboratively to help people remain healthier within their own communities. Key Responsibilities Provide compassionate, patient-centered care to adults and seniors through telehealth and in-person encounters. Conduct visits in patient homes, community settings, outreach programs, and other locations as needed. Perform health assessments, evaluate and diagnose conditions, develop treatment plans, and provide appropriate follow-up care within scope of practice. Provide preventive care and health education, including identifying gaps in care and opportunities for early intervention. Support patients with chronic conditions through ongoing assessment, education, medication management, and coordination of care. Assist individuals transitioning from hospitals or other healthcare settings back into their homes and communities. Identify medical, behavioral health, social, and access-related needs that may affect a patient's health and ability to follow a treatment plan. Collaborate with physicians, behavioral health professionals, care coordinators, community organizations, and other members of the Centered Care Wellness team. Help patients understand their health conditions, medications, treatment recommendations, and preventive health needs. Connect patients with appropriate healthcare and community resources when additional support is needed. Participate, as appropriate, in community health initiatives, outreach events, preventive health programs, and nonprofit-funded programs. Maintain timely and accurate clinical documentation and comply with applicable clinical, regulatory, and organizational requirements. Qualifications Graduate degree from an accredited Nurse Practitioner program. Current, unrestricted California Nurse Practitioner (NP) certification/licensure and all required credentials to practice within the position's scope. National board certification appropriate to the candidate's specialty, as required for their practice authority and role. Experience in primary care, family practice, adult health, geriatrics, preventive health, chronic disease management, community health, or a related area preferred. Experience working with underserved, medically complex, or socially vulnerable populations is highly valued. Comfortable providing both telehealth and in-person care. Must reside in or near the San Francisco Bay Area and be available to travel to Antioch and surrounding communities for patient and community visits. Strong clinical judgment, communication, and interdisciplinary collaboration skills. Commitment to culturally responsive, respectful, and patient-centered care. Interest in nonprofit and community-based healthcare. Spanish or other languages commonly spoken within the communities we serve are a plus. The NP We're Looking For We are looking for someone who sees the patient beyond the diagnosis. Many of the individuals we serve face challenges that make accessing traditional healthcare difficult. Sometimes the most effective way to close that gap is to bring care into the community rather than expecting the community to come to us. You should be comfortable talking with patients about their health in understandable terms, identifying barriers that may prevent them from following a care plan, and working creatively with the broader care team to find practical solutions.
