In this vital role, you will provide advanced clinical care focused on the assessment, treatment, and management of complex wounds. You will perform comprehensive patient assessments, develop personalized wound treatment plans, and support patient recovery through evidence-based practices. Key Requirements Must be Medicare certified Must have proven experience in wound care A plus if Medi-Cal certified and/or credentialed with managed care plans Responsibilities Conduct thorough wound assessments and physical examinations to evaluate wound status, underlying conditions, and overall health. Develop and implement personalized treatment plans for a wide range of wounds, including burn wounds, diabetic wounds, pressure ulcers, surgical wounds, traumatic wounds, and venous ulcers. Prescribe medications, advanced wound therapies, dressings, and treatments in accordance with state regulations and collaborative practice agreements. Perform wound care procedures such as debridement, suturing, wound packing, IV insertions, catheterizations, and other sterile techniques with precision. Manage acute and chronic wound-related conditions, including complications in geriatric, diabetic, and post-surgical patients. Utilize electronic health record (EHR) systems (Epic, Cerner, eClinicalWorks, or similar) for accurate documentation, ICD-10 coding, billing, and clinical notes. Collaborate closely with physicians, specialists, nurses, and therapists to coordinate comprehensive wound care. Provide telehealth consultations when appropriate while maintaining full HIPAA compliance. Educate patients and caregivers on wound prevention, self-care, and recovery strategies. Skills & Qualifications Demonstrated clinical experience in advanced wound care and management of complex wounds. Medicare certification required; Medi-Cal certification and/or managed care plan credentialing strongly preferred. Proficiency with EMR/EHR systems and accurate medical documentation/coding. Strong knowledge of wound healing physiology, pharmacology, medical terminology, and sterile technique. Experience working with diverse populations, including seniors, patients with diabetes, and those in assisted living or home settings. Excellent communication, health coaching, and patient education skills. Ability to work both independently and as part of a collaborative care team in outpatient, home, or mobile settings.
