Wound Care Provider Wound360 is seeking an experienced Wound Care

NP/PA/MD/DO

to provide hands-on wound care services to home health patients in Vallejo, and surrounding areas . This is a mobile home health role , where the provider will travel directly to patients' homes to assess wounds, perform appropriate wound care procedures, and coordinate care with the patient's healthcare team. About the Role We are looking for a skilled and dependable provider who is comfortable working independently and providing hands-on wound care in the home setting . The provider will be responsible for visiting assigned home health patients, evaluating their wounds, performing appropriate treatments and procedures, documenting care, and communicating recommendations to the patient's care team. Responsibilities Conduct wound care visits in patients' homes throughout Sacramento and nearby areas. Assess and evaluate chronic, surgical, and other complex wounds. Perform sharp/surgical debridement when clinically appropriate. Perform dressing changes and provide wound treatment recommendations. Manage and assess wound vacs and other wound care therapies. Provide feeding tube/G-tube management as applicable. Assess patients for wound healing progress and identify changes that may require additional medical attention. Coordinate with home health nurses, physicians, caregivers, and other members of the patient's care team. Provide clear treatment recommendations and follow-up plans. Complete accurate and timely documentation in the EMR. Travel between assigned patient homes as needed. Qualifications Active California NP, PA, MD, or DO license . Strong clinical and hands-on experience in wound care and sharp/surgical debridement . Home health experience preferred. Experience with SNF, home health, hospice, LTAC, or other post-acute settings preferred. Experience with wound vacs, feeding tubes/G-tubes, and skin grafts preferred. Prior experience working with a wound care company or wound care program is a plus. Comfortable traveling to multiple patient homes within the Sacramento area and surrounding communities. Able to work independently while maintaining strong communication with the patient's care team. Professional, dependable, and comfortable building rapport with patients and families.

Availability:

must be available at least 2 days per week to see patients

Compensation & Other Details Compensation:

$150-$200 per visit , open to further discussion with the practice.

Mileage reimbursement:

To be discussed with the practice.

Schedule:

Based on assigned home health patients and geographic coverage.

Location:

Vallejo, CA and nearby areas.
