2996669468-2 Apply for this job now Job Description Hartford Healthcare seeks a Primary Care Birth+ Family Nurse Practitioner or Physician Assistant to provide comprehensive primary care across the lifespan in a collaborative, patient-centered setting. In this role, you will conduct exams, manage chronic and acute conditions, order and interpret tests, prescribe medications, and deliver preventive care and counseling. Responsibilities Provide comprehensive primary care for birth+ and family patients, including assessment, diagnosis, and treatment Perform wellness exams, manage chronic conditions, and provide preventive care and health education Order, interpret, and follow up on diagnostic tests and lab results Collaborate closely with physicians, nurses, and interdisciplinary team members to coordinate care Document patient histories, care plans, and outcomes in the electronic health record (EHR) Prescribe medications and therapies in accordance with state and federal guidelines Counsel patients and families on health promotion, disease prevention, and treatment options Participate in quality improvement initiatives and adhere to clinical best practices and protocols Maintain compliance with regulatory, safety, and infection control standards Engage in ongoing professional development and contribute to a culture of teamwork and compassion Required Skills Primary care clinical practice Family practice across lifespan Newborn and pediatric assessment Chronic disease management Acute care evaluation and treatment Ordering and interpreting diagnostic tests Prescriptive authority and pharmacology Electronic Health Records (EHR) documentation Patient and family health education Interdisciplinary care coordination
