In this role, you'll provide comprehensive primary care, manage chronic conditions, and promote wellness for diverse patient populations. Responsibilities Provide comprehensive primary care to patients across the lifespan, including health promotion and disease management Conduct patient assessments, order and interpret diagnostic tests, and develop evidence-based treatment plans Prescribe medications and therapies in accordance with state and federal regulations Coordinate care with physicians, specialists, and interdisciplinary team members Educate patients and families on disease prevention, medications, and self-management strategies Document all encounters accurately in the electronic health record (EHR) Participate in quality improvement initiatives and population health management Collaborate in care transitions, referrals, and follow-up to ensure continuity of care Maintain compliance with organizational policies, clinical protocols, and regulatory standards Engage in ongoing professional development and contribute to a culture of teamwork and compassion Required Skills Primary care clinical assessment Chronic disease management (e.g., diabetes, hypertension) History taking and physical examination Ordering and interpreting labs and imaging Medication management and prescribing Use of electronic health records (EHR) Preventive care and wellness counseling Care coordination and referral management Patient and family education Evidence-based clinical decision-making
