Advanced Practice Clinician (Nurse Practitioner / Physician Assistant)
Location:
Hartford County, CT Schedule:
Monday through Friday, 8:00 AM to 5:00
PM Duration:
1 year Contract with potential for permanent hire

Yes, between 2 facilities during the workday Overview We are seeking an experienced Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to provide comprehensive, patient-centered care for high-risk individuals residing in senior living and long-term care settings. This role is part of an interdisciplinary care team focused on delivering longitudinal, coordinated care that improves health outcomes and enhances quality of life. This position offers a predictable weekday schedule with no on-call responsibilities, weekends, or holidays, supporting a strong work-life balance. Responsibilities Conduct comprehensive health assessments and evaluations. Diagnose, treat, and manage acute and chronic medical conditions. Address behavioral health concerns as appropriate. Maintain accurate and timely clinical documentation, including ICD-10 diagnosis coding. Collaborate with physicians, nurses, social workers, and other members of the interdisciplinary care team. Participate in advanced care planning and goals-of-care discussions with patients and families. Provide patient and caregiver education regarding disease management, treatment plans, and preventive care. Develop and implement individualized care plans to address complex healthcare needs. Monitor patient progress and adjust treatment plans as necessary. Required QualificationsNurse Practitioner Active and unrestricted Nurse Practitioner license in the state of practice or ability to obtain prior to start date. Graduate of an accredited Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) program. Current national board certification as one of the following: Family Nurse Practitioner (FNP) Adult Nurse Practitioner (ANP) Gerontology Nurse Practitioner (GNP) Adult-Gerontology Acute Care Nurse Practitioner (AGACNP) Physician Assistant Active and unrestricted Physician Assistant license in the state of practice or ability to obtain prior to start date. Graduate of an accredited Physician Assistant program. Current NCCPA certification. Additional Requirements Current DEA registration and prescriptive authority. Ability to obtain a collaborative practice agreement, if required by state regulations. Reliable transportation for travel between patient care locations. Ability to perform physical aspects of the role, including: Lifting and carrying up to 30 pounds. Navigating stairs and varying facility environments. Sitting, standing, bending, and kneeling to conduct patient assessments. Compliance with applicable vaccination, health screening, and credentialing requirements. Ability to successfully complete required pre-employment screenings. Preferred Qualifications At least 1 year of clinical practice experience; long-term care, skilled nursing, or senior living experience preferred. Experience caring for patients with complex medical, behavioral, social, and functional needs. Knowledge of geriatric medicine and chronic disease management. Experience conducting advanced illness, palliative care, and end-of-life discussions. Strong computer proficiency and experience documenting in electronic medical record (EMR/EHR) systems. Excellent communication, collaboration, and patient education skills.

Antoine Residence Travel Requirements:
Travel occurs during the workday between assigned facilities. Coverage includes approximately two locations.
