This is an opportunity for a Spanish-speaking Nurse Practitioner or Physician Assistant to provide comprehensive care to Medicare-aged patients in a supportive outpatient setting. What Makes This Opportunity Different Full-time employed position 100% outpatient primary care Monday-Friday schedule (8:00 AM-5:00 PM) No evenings, weekends, holidays, or call responsibilities 13-15 patients per day on average Longer appointment times that support personalized, relationship-based care Collaborative team including physicians, nurses, medical assistants, and care coordinators Value-based care model focused on improving patient outcomes and reducing unnecessary hospitalizations For immediate consideration email CV/resume to ssager@provenirusa.com Your Responsibilities Provide comprehensive primary care for adult and senior Medicare Advantage patients Evaluate, diagnose, and treat acute and chronic medical conditions Focus on preventive care, wellness visits, and chronic disease management Educate patients and families on treatment plans and healthy lifestyle choices Coordinate care with the interdisciplinary healthcare team Maintain accurate and timely clinical documentation Qualifications Graduate of an accredited Nurse Practitioner or Physician Assistant program Active Florida APRN or PA-C license in good standing National Board Certification required ( AANP, ANCC, or NCCPA ) Fluent in both Spanish and English with the ability to communicate effectively with patients 1-2+ years of outpatient primary care experience strongly preferred Experience caring for Medicare Advantage, geriatric, or senior patients highly preferred Experience with value-based care, chronic disease management, and preventive care is a plus Strong commitment to compassionate, patient-centered care and building long-term patient relationships Please TEXT Sierra at 210-591-7125 to set up a time to talk!
