Responsibilities:
Conduct outpatient psychiatric evaluations, including intake assessments & follow-up assessments, diagnosis, treatment, and ongoing support for individuals with mental health conditions. Create, communicate, and carry out treatment plans with the patient. Complete full and appropriate documentation to support professional billing for services. Interpret test results to diagnose any health issues and provide feedback to patients. Educate and counsel patients regarding medication compliance. Prescribe medications when clinically necessary. Communicate with patients regarding diagnosis and treatment plan. Communicate with and make referrals to PCPs and/or specialists as appropriate. Provide services at community partnership locations. Maintain required licensure, certifications, and CME credits. Other duties as assigned.
Daily volume expectations:
10-15 patients Full-time, M-F, 8 am to 5 pm No call
Qualifications:
Must possess an active & unrestricted license to practice in the state of Florida. Must possess an unrestricted DEA license in Florida. Preferred to possess a current board certification by a national certifying body as a Board-Certified Nurse Practitioner or Physician Assistant. Eligible for credentialing with all major insurance plans and to participate in Medicare. Excellent communication, patient relations, time management skills, and safety awareness are required. Computer literate and experienced with Electronic Medical Records.

Dr. Sherazy specializes in complex psychiatric and addiction cases, including treatment-resistant mood disorders, and offers advanced treatments such as TMS and Spravato. Dr. Rodriguez brings more than 10 years of experience and focuses on thoughtful, personalized care for mood, anxiety, psychotic, substance use, and ADHD conditions. Patients describe both doctors as compassionate, professional, attentive, and willing to listen. Our team is committed to providing personalized care and giving patients options when traditional treatment has not provided enough relief. Pay:
$130,000.00 - $140,000.00 per year
