Job Description
Job Description Help for Job Description. Opens a new window. Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities
- Full-time providers care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day
- Manage a panel of 200 patients (approximately 12-16 patients per/day with 14 being the avg.)
- Ability to design and manage your schedule
- Develop and implement care plans appropriate to patients and their desires
- Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social Workers
- Access to diagnostics such as labs and x-rays as needed
- Collaborate with community partners within home care, hospice, and other resources as appropriate
