Job Description

We're expanding our team that delivers patient-focused care to residents in skilled nursing facilities around DeKalb, IL. If you're a compassionate Nurse Practitioner who enjoys working with older adults in a collaborative, team-based setting, we'd love to hear from you. Opportunities also exist in MA, MI, NH, NC, OH, and WI. What you'll be doing Provide daily rounds at skilled nursing facilities, managing both acute and chronic conditions. Develop and adjust treatment plans, prescribe medications, and coordinate care with facility staff and families. Work efficient shifts (typically six hours) with rounds beginning between about 7:00 a.m. and 10:00 a.m. Document care using a streamlined EHR enhanced by AI Scribe and voice dictation on a company-provided iPad with LTE. Use UpToDate® for current, evidence-based clinical guidance. What we offer Full-time roles with flexible scheduling and no overnight or on-call obligations. Who we're looking for Active, unrestricted Nurse Practitioner license and current board certification (ANCC, AANPCB, PNCB, AACN, or equivalent). Prior experience in skilled nursing, long-term care, geriatrics, or post-acute care is preferred. Compassion for older adults, strong communication skills, and a team-oriented approach to care.
