Advanced Practice Clinician Nurse Practitioner or Physician AssistantPosition Overview Our physician-led practice in Niles, Illinois, is seeking a dependable and personable Advanced Practice Clinician—Nurse Practitioner or Physician Assistant—to join our team. The clinician will primarily provide outpatient primary care one day per week, with additional hours assisting in our urgent care center and performing home visits. This position is well suited for a clinician who values continuity of care while remaining comfortable evaluating acute illnesses and injuries. We are a small, collaborative team that includes a physician, part-time nurse practitioner, nurse, medical assistant, and front-desk support. Responsibilities Evaluate and treat patients in an outpatient primary care setting Diagnose and manage common acute and chronic medical conditions Perform age-appropriate histories, physical examinations, preventive-care assessments, and follow-up visits Order and interpret laboratory studies, imaging, and other diagnostic testing Prescribe medications within the clinician's scope of practice Provide patient education regarding diagnoses, medications, preventive care, and disease management Assist with urgent care coverage as needed Perform home visits Recognize patients requiring a higher level of care and coordinate appropriate referrals or emergency evaluation Perform common outpatient procedures based on training and competency Document encounters accurately and promptly in the electronic medical record Use appropriate ICD-10 and CPT coding to support accurate billing Collaborate with the physician and clinical support staff to coordinate patient care Maintain patient privacy and comply with HIPAA and applicable professional standards Qualifications Current Illinois Nurse Practitioner or Physician Assistant license in good standing Current national board certification Active Illinois controlled-substance license and DEA registration, or eligibility to obtain them Current BLS certification Primary care, family medicine, internal medicine, emergency medicine, or urgent care experience preferred Emergency medicine or urgent care experience is a strong plus Comfortable caring for adult and geriatric patients; pediatric experience is beneficial Ability to evaluate both acute and chronic conditions and identify patients requiring escalation of care Strong clinical judgment, communication, and documentation skills Professional, reliable, organized, and able to work effectively in a small-team environment Comfortable using an electronic medical record Valid driver's license and reliable transportation for scheduled home visits Schedule Primary care responsibilities approximately one day per week Additional urgent care shifts and scheduled home visits Specific days and hours will be discussed based on practice needs and candidate availability What We Offer Friendly and professional physician-led environment Direct access to physician consultation and support Established clinical and administrative support staff Variety of primary care and urgent care experience Opportunity for the position and schedule to grow based on mutual fit and practice needs Qualified Nurse Practitioners and Physician Assistants are encouraged to apply with a current resume and a brief description of their relevant primary care, emergency medicine, or urgent care experience.

Pay:
