Exempt Position SummaryThe Nurse Practitioner coordinates the planning, implementation and on-going monitoring and evaluation of patients of all age ranges, under the supervision of a collaborating licensed physician. Responsibilities include examining, diagnosing, treating patient conditions, educating patients and family members about medical options, prescribing medications and treatments, and performing other duties to support patients. General Duties, Tasks and ResponsibilitiesWork independently with other members of the medical team that take part in an interdisciplinary team that provides primary care to employees and patientsPrescribe medications following the clinics policy, formulary and relevant regulations of the StatePerforms and documents mental health assessments, consults as needed with the provider/practitioner concerning the plan of care.

Maintains current knowledge of community resources, how to access them and the regulations for access to them.

Communicates effectively in both the patient chart and orally with the staff so as to ensure that can be understood and executed by others as appropriate.

Complies with all established safety procedures to ensure a safe environment for patients, visitors and staff.

Participates in performance improvement activities.

Performs other duties as assigned.
