POSITION SUMMARY
The Midlevel Practitioner delivers clinical care in assigned specialty areas, functioning as a fully accountable colleague to physicians and clinic teams through collaborative care. This role supports integrated care coordination (behavioral & oral health), and value-based transformation efforts, promoting and patient's health by prescribing, providing, delegating, evaluating and coordinating comprehensive care for patients of all ages, genders, cultures, and backgrounds; including physical and psycho-social support for patients and families. The Midlevel Practitioner may also coordinate student nursing staff and clinic volunteers. This position is responsible for always maintaining patient/client confidentiality.

DUTIES & RESPONSIBILITIES
Advance improvements in patient well-being through accurate and timely diagnosis and delivery of relevant, high-quality medical care. Ensure the delivery of competent, accurate, medical care and treatment to all patients. Meets or exceeds all measurable standards of care, including overall clinical, quality, and productivity goals. Ensures accuracy, efficiency, and confidentiality with patient charting and related recordkeeping and administrative functions. Ensures the delivery of all medical treatment and care is consistent with regulations, accreditation, and professional standards, including clinical policies and quality initiatives. Understands and ensures all practices and procedures related to medical services are consistent with contracted protocols and procedures. Contributes to quality assurance and improvement programs, and patient centered medical home transformation success. Develops and ensures positive relationships with patients and their families. Obtain commitments from patients for an effective plan of care, and ensure patient services are delivered in a caring and professional manner. Develops and ensures ongoing, positive relationships and collaboration with other physicians, nurses, administrative and clinic operations staff. Ensuring job performance, personal conduct and communications always represent the organization in a highly professional manner. Upholds and ensures compliance and attention to all corporate policies and procedures as well as the overall mission and values of the organization. Oversee overall comprehensive care including physical assessments, evaluations, diagnoses, initiating and interpreting labs and x-rays; performing medical procedures. prescribing and referring patients for specialized consultation; assisting Physicians in medical procedures as requested and using prescriptive authority to write prescriptions Completes progress notes on patients' charts indicating patient status and treatment Conducts follow-up patient care. Provides health education to patients, families, and the public. Supervises and/or coordinates patient care and support staff within the health care center. Teaches and trains illness prevention. Actively participates in community health education. Performs emergency life-saving procedures in cases such as cardiac arrest, respiratory arrest, massive hemorrhage, or similar emergencies. Participates among front line medical providers in emergency disaster services.
ESSENTIAL FUNCTIONS/KEY COMPETENCIES
Build relationships and excel at customer service, especially in managing difficult clients and patients, and problem-solving skills Demonstrates knowledge and competency in the practice of medicine and associated charting requirements. Strong attention to detail and accuracy. Ability to utilize computers for data entry and information retrieval. Excellent verbal and written communication skills. Ability to implement and evaluate operational and administrative processes. Other duties as assigned
REQUIRED KNOWLEDGE, SKILLS AND EXPERIENCE
Graduate from an accredited college or university with a Master of Nursing or advanced degree; advanced degree and relevant certification in medicine. Knowledge of the principles and practices of preventative medicine. Demonstrated expertise in relevant medical practices, protocol, trends and best practices in clinical areas assigned. Experience/Knowledge of JCAHO accreditation process and requirements, as well as all federal, state and local regulations and standards associated with the delivery of care in a community health center environment. Ability to manage multiple responsibilities and emergency situations successfully
Occasional lifting of 40 - 50 lbs and pushing of 5-20 lbs
Sitting, standing and walking approximately 80% of the time
A medium to high level of manual dexterity required
Bending and reaching approximately 20% of the time
Ability to be mobile throughout the region required Overtime/extended work hours may be required.
