09/05/2026 Job Description Job Description Nurse Practitioner or Physician Assistant - Skilled Nursing FacilityAbout the Role We're looking for an experienced, autonomous Nurse Practitioner or Physician Assistant to join our clinical team full time, providing primary and episodic care to residents in skilled nursing facilities. This is an independent role built around continuity - you'll manage your own panel and build long-term relationships with patients and their families. What's In It For You You build your own schedule. You decide the days and hours that work for you. Keep it consistent once it's set, and the rest is yours. 6-hour shifts. Rounds generally start in the morning (7-10 a.m.). No on-call, no overnights, ever. Chart from home. Wrap up your documentation wherever you want - you're not tied to the facility once your rounds are done. Real earning potential. Productivity-based compensation - the more you see, the more you make. High-volume providers seeing 24 patients in a shift can earn up to $624/day. Long-term retention incentive up to $50,000 on top of your pay.

Your Day-to-Day You'll see roughly 16-20 patients a day - a mix of long-term chronic care and acute issues. That means annual wellness visits and H&Ps, managing chronic and co-morbid conditions, responding to acute changes, reviewing meds and orders, and coordinating with collaborating physicians, specialists, and facility staff to keep care seamless. You'll also stay in touch with families and join care conferences where it makes sense. Qualifications Board certification required (e.g., AGNP, FNP for Nurse Practitioners; PA-C for Physician Assistants) - active and unrestricted. Valid, unrestricted Nurse Practitioner or Physician Assistant license in the state of operation. Master's degree in Nursing (Nurse Practitioner) or from an accredited Physician Assistant program - required for the corresponding credential. Skilled nursing or long-term care experience strongly preferred. Strong independent clinical judgment and the ability to move quickly in a fast-paced setting.
