Job Summary:

Delivers primary care commensurate with training and monitors clinical performance to ensure medical services and operations are in compliance with all applicable regulatory and licensing agencies.

Duties/Responsibilities:

Meet all specific requirements and performs duties as outlined in individual contracts and amendments. Performs physical examinations and maintains records appropriately. Performs physical assessments, recognizing the range of normal and managing the cessation of abnormalities and initiation of treatments. Orders diagnostic testing as needed, including x-rays, lab work, screening exams, and other specialized testing as needed and is responsible for notification to the patient of test results and arranging appropriate follow-up testing or consultation when necessary. Develops patient history and assess risk factors, including hypertension, diabetes, smoking, substance and alcohol abuse, and other medical, physiological, and psychological conditions. Includes patient and family members in plan of care, provides educational counseling and advocacy about health related issues. Promotes and participates in community health education and outreach programs. Collaborates with physicians, nurses, physician assistants, and other providers in the group practice and community. Consults with physician on case load development and more complicated physical problems. Assists in ongoing record review and performance improvement studies. Participates in health center quality assurance program and follows its developed protocols and guidelines. Practices universal precautions and adheres to OSHA and workplace safety guidelines. Completes all charting and medical record documentation including progress notes, problems lists, medication lists, allergy documentations, dictations, completion of insurance forms, and quality assurance documentation, in a timely manner. Assists in the development and implementation of the health center's health care plan. Observes patient confidentiality and obtains appropriate release of information when necessary. Adheres to HIPAA requirements.

Skills/Knowledge/Abilities:

Understanding of ethical implications to the staff/patient relationship. Must adhere to confidentiality and HIPAA policies and procedures. Demonstrate awareness of, and value for, cultural competence. Strong commitment to serving the underserved and vulnerable populations of the local community. Must demonstrate a commitment to the mission of Covered Bridge Healthcare. Knowledge of the principles and practices of preventative medicine. Knowledge of the state and federal laws pertaining to medicine and to the community health centers. Basic understanding of health clinic processes and operations. Understanding of Federally Qualified Healthcare Center (FQHC) processes and operations (preferred).

Education/Experience:

Graduation from an accredited Nurse Practitioner/Physician Assistant Program. Current Nurse Practitioner or Physician Assistant Certification in Family Medicine. State of Michigan Medical Licensure. Current DEA registration and Controlled Substances Licensure (preferred). Basic Life Support (BLS)

$90,000.00 - $120,000.00 per year
