Rather than seeing dozens of unrelated patients in a clinic, you'll manage your own panel while caring for residents in assisted living , memory care , skilled nursing , and other senior living settings. You'll diagnose and treat acute illness, manage chronic disease, perform comprehensive assessments, coordinate care after hospitalization, and collaborate closely with nurses, therapists, social workers, and facility leadership to improve patient outcomes. You'll have the opportunity to truly influence how care is delivered while becoming a trusted clinical resource within each community you serve. What You'll Be Doing Provide comprehensive primary care throughout senior living communities Diagnose, treat, and manage acute and chronic medical conditions Develop individualized treatment plans Coordinate transitions of care following hospitalization Collaborate with physicians and interdisciplinary healthcare teams Educate patients, caregivers, and community staff Participate in quality improvement initiatives Help reduce unnecessary emergency room visits and hospitalizations Clinical Backgrounds That Thrive Here We welcome Nurse Practitioners with backgrounds in: Primary Care Family Practice Internal Medicine Geriatrics Long-Term Care Skilled Nursing Post-Acute Care Home Health Hospice & Palliative Care Community-Based Medicine What We're Looking For Required Qualifications Active Minnesota Nurse Practitioner license Strong assessment, diagnostic, and clinical decision-making skills Ability to practice independently while collaborating with physicians Passion for caring for older adults Reliable transportation and willingness to travel throughout the Saint Cloud region Preferred Qualifications Advanced wound care experience Chronic disease management expertise Experience serving seniors in community-based settings Wound Care Certification (WCC) is a plus Build a Career That Fits the Way You Want to Practice.
