• Hazelwood is powered by 3 Physicians, 2 Advanced Practitioners, and an engaged support staff focused on quality patient care.

Previous Advanced Practitioner Primary Care experience is preferred. This is a great opportunity to step into a collaborative, patient-centered practice with a strong referral base and excellent resources.

This Opportunity Offers:

Hours:

Monday-Fri 8am-4:30pm

Full-time position

Outpatient Clinic

Treating patients of all ages

Managing Chronic and Acute conditions

No weekend or call requirement

System-wide Epic EMR
